FDA Approves First Oral Carbapenem Antibiotic for Complicated Urinary Tract Infections

U.S. Food and Drug Administration (FDA) approved the oral antibiotic Utebzi (tebipenem pivoxil) for complicated urinary tract infections (cUTIs), developed by GSK and Spero Therapeutics. It is the first oral carbapenem antibiotic approved in the United States, targeting multidrug-resistant infections that previously required intravenous carbapenems. About three million cUTI cases occur annually in the United States, with roughly one-third experiencing treatment failure due to antibiotic resistance. Trial results from the PIVOT-PO study showed noninferiority versus IV imipenem-cilastatin, with clinical success rates of 58.5% versus 60.2%. Safety was comparable between treatments and the trial was stopped early for efficacy. The approval follows a 2022 licensing agreement between the companies enabling development and commercialization of the drug in global markets outside Asia. It provides a new oral treatment option that may reduce hospital IV antibiotic use and ease the burden of complicated urinary tract infections. Overall, this represents a significant clinically therapeutic advance.
