The Future of Immunotherapy in Prostate Cancer: Learning From Failed Trials

For years, immunotherapy has shown limited success in prostate cancer, with six phase 3 checkpoint inhibitor trials failing to demonstrate significant benefits. However, researchers are now studying rare patients who experienced exceptional responses to uncover why some tumors can be eliminated while most remain resistant.

Studies suggest that prostate cancer is often an immunologically “cold” tumor, lacking active immune cells needed for checkpoint inhibitors to work. Research using single-cell analysis has revealed that specific immune-suppressive macrophages, particularly SPP1-high macrophages, contribute to treatment resistance by blocking T-cell activity. These findings highlight new therapeutic targets, including strategies aimed at disrupting adenosine signaling and altering the tumor microenvironment.

One remarkable case involved a patient with advanced prostate cancer, mismatch repair deficiency, and extremely high tumor mutation burden who achieved complete remission after pembrolizumab treatment. Analysis of his immune cells revealed expansion of unique NK-like T cells and double-positive T cells, which may have driven the elimination of cancer cells.

New approaches, including CD3-targeting bispecific T-cell engagers, radioligand therapy combined with immunotherapy, and nanoparticle-based treatments, are being developed to overcome immune resistance. Although challenges remain, researchers believe that lessons from failed trials and exceptional responders are opening a new era in prostate cancer immunotherapy, moving toward more personalized treatments based on biomarkers and tumor biology.
