Managing Male Urinary Incontinence: The DIAPERS Guide

Geriatric urinary incontinence is common but often overlooked, especially in men. Dr. Neil M. Resnick, a leading expert in geriatric medicine, emphasizes that it’s usually not due to bladder issues but to reversible medical causes. To help diagnose these, he created the DIAPERS mnemonic: Delirium, Infection, Atrophic urethritis, Pharmaceuticals, Excess urine output, Restricted mobility, and Stool impaction.

Each factor can trigger or worsen incontinence in aging men. For example, medications like sedatives, antihistamines, and diuretics are common culprits. Overhydration (like following the “8 glasses of water” myth), limited mobility due to pain or foot issues, and even constipation can lead to bladder problems.

Dr. Resnick stresses that urinary incontinence is never a normal part of aging, and men shouldn’t feel embarrassed to bring it up with their doctor. Most cases can be improved or even cured with simple medical or lifestyle adjustments. Only a small percentage of elderly patients need surgery.

The takeaway? Incontinence in older men is often treatable and preventable. Recognizing and addressing the causes can improve not just bladder control but also quality of life, confidence, and overall health. Don’t stay silent—talk to your doctor.
